Application instructions appear on that page, along with links to online (preferred) and printable application forms. Please note that the program application deadline is earlier than the date listed on the written application form.
All students should apply on the Graduate College Application. On the application form, enter “Oral Science/Certificate in Oral and Maxillofacial Pathology ” or “Oral Science/Certificate in Oral and Maxillofacial Radiology ” for Department or Program (code 151 on the written application), “Pathology” or “Radiology” for Subarea of Interest or Specialization, and “MS/CER” for the degree desired (MS/thesis on the written application).
Applicants must also send the following materials to the Department of Oral Pathology, Radiology, and Medicine: three letters of recommendation, a statement of purpose, curriculum vitae, official academic records/transcripts from all institutions attended and Application Supplement.
Students from countries whose primary language is not English must present evidence of satisfactory performance on the Test of English as a Foreign Language (TOEFL) from the Educational Testing Service (the University’s institutional code is 6681) and the Test of Spoken English (TSE). Once recommended for admission, international students must send a Financial Statement.
Applications for the program will be accepted at any time. Completed applications received by September 1 will be considered for admission for the entering class starting the following July. Final decisions on acceptance of applicants who meet the requirements for admission are made by the department faculty. A personal interview is required. Applicants accepted for admission are required to participate in a criminal background check.
